Mentor on Discharge

What is "Mentor on Discharge"?

The MoD (Mentor on Discharge) program has been shown to reduce the re-hopitalization rate of psychiatric patients in an acute care psychiatric hospital by over 70%. The MoD program has been implemented by NAMI Alameda County South. With a Kaiser Permanente Community Benefit Grant, we are now providing mentor services to John George Psychiatric Hospital and Telecare Heritage Hospital.

Since the MoD program is not a NAMI Signature Program, we cannot use NAMI general funds for it. However, with the Kaiser Permanente Community Benefits Grant and a private donations specifically for the MoD Program we plan to continue this valuable program. Thank you grantors and donors.

If you would like to support this program, it costs an average of $1,000 to support each participant with a mentor in the MoD program. Please make a donation to NAMI ACS specifying that your funds are to be used for our MoD program.
